Information you choose to send

The enquiry form sends your details directly to info@mobilephysio.co.uk through FormSubmit, a third-party form delivery service. This includes your name, phone number, optional email address, postcode, preferred day and message. You do not need to open an email application. The website does not store enquiries in its own database. FormSubmit processes the submission to deliver it by email; its own privacy policy applies to that processing. Please do not include sensitive medical information. See FormSubmit's privacy policy.

How information is used

Enquiry information is used to respond to you, check whether a home visit is appropriate, confirm service-area coverage and arrange an appointment. Please avoid including detailed or sensitive medical information in your first website enquiry.

Clinical records

If you become a patient, information required for your care and professional record-keeping will be handled separately as part of the clinical service. You will be given relevant information about this when arranging care.

Technical information

The website hosting provider may process basic technical information needed to deliver and secure the site, such as IP address, browser details and access logs. This website does not currently use advertising trackers or analytics cookies.
